ADULT NEW PASSPORT This application is for you if:
	You are over 16 years old as of today AND
	You have never had a U.S. Passport issued to you before OR
	You were under 16 years old on the date your most recent passport was issued OR
	You haven't had a U.S. Passport issued for over 15 years OR
	You have been instructed to follow this procedure from It's Easy

CHILD NEW PASSPORT This application is for you if:
	You are 15 years old or younger as of today AND
	You have not lost a U.S. Passport that would be valid today

PASSPORT RENEWAL This application is for you if:
	You were 16 years old or over on the date your most recent passport was issued AND
	Your passport was issued within the past 15 years AND
	Your passport has not been damaged in anyway

ADULT LOST OR STOLEN PASSPORT This application is for you if:
	You are over 16 years old as of today AND
	You have lost your most recently issued U.S. Passport AND
	Your lost passport would still be valid today (or you think it might still be valid today)

CHILD LOST OR STOLEN PASSPORT This application if for you if:
	You are 15 years old or younger as of today AND
	You have Lost a U.S. Passport that would be valid today (or you think it might still be valid today).

ADULT MUTILATED OR DAMAGED PASSPORT This application is for you if:
	You are 16 years old or older as of today AND
	Your current passport has been damaged. (If you are not sure about the severity of the damage give our office a call we will be happy to provide you with assistance).

CHILD MUTILATED OR DAMAGED PASSPORT This application is for you if:
	You are 15 years old or younger as of today AND
	Your current passport has been damaged. (If you are not sure about the severity of the damage give our office a call we will be happy to provide you with assistance).

CHANGING YOUR NAME This application is for you if:
	Your current passport has not been damaged in any way AND
	Your name changed AFTER your most recent passport was issued AND
	Your PASSPORT WAS ISSUED LESS THAN 1 YEAR AGO. (Passports issued over a year ago must be done as passport renewals).

2ND PASSPORT This application is for you if:
	You have a currently valid passport, but you need a second valid passport due to the nature of your travel. Call the It's Easy staff to double check that is the application you should be applying for.

REPLACING LIMITED VALIDITY PASSPORTS This application is for you if:
	Your current passport has not been damaged in any way AND
	Your passport was issued for a term of less than 5 years (usualy a one year passport) AND
	Your passport was issued within the past year. AND
	If your passport was limited because it replaced a lost or stolen passport than it needs to have been issued OUTSIDE the United States.

	Passports issued over a year ago must now be done as Renewals
	Replacements for lost or stolen passports issued INSIDE the United States must be done as New Passports.

EXTRA PAGES This application is for you if:
	Your current passport has not been damaged in any way AND
	You have used up over half of the pages of your current passport.

	You will get the same passport back, with the same number, and expiration date. If your passport expires within the next year and the applicant is for an adult we would recommend that you do a passport renewal instead of adding pages. You can request the new passport to be a larger book.
