ONE APPLICATION FORM

Include one DS11 Application Form completely filled in.

PROOF OF CITIZENSHIP
   

Proof of U.S. Citizenship. You will need to provide one of the following.

Original Government Issued Birth Certificate. Hospital and religious certificates are not accepted. OR

Previous U.S. Passport issued for full term (5 to 10 years) that has not been damaged in any way. OR

Original Naturalization Certificate

   
NOTES

Photocopies and notarized photocopies are NOT accepted, all certificates must be originals

The birth certificate must have the applicant's first name listed on it

3.
PROVIDING IDENTIFICATION
   

You will need to provide:

A clear copy of your U.S. Driver's License. You will have to have the original with you when you sign the application form.

   
EXTREMELY IMPORTANT

The issue date on your license must be over 6 months ago. If you have had your license renewed, modified or replaced recently make sure you check the issue date!

State Ids and permits are NOT accepted - ONLY a U.S Driver's License will be acceptable.

4.
PASSPORT PHOTOS
Submit two identical passport photos taken within the past few months.
   
These should be 2x2, white background, unaltered pictures that meet the U.S. Governments PASSPORT PHOTO REGULATIONS.
   
Kinkos, AAA, and Mailboxes Etc are examples of stores that take passport photos. If you don't have one of these stores located near you, try looking in your local Yellow Pages under "Photographers - Passport".
Find a KINKOS near you. (Be sure to ask if they take passport photos - now that Kinkos has merged with FedEx not all their locations take passport photos).
Find a AAA near you (be sure to ask that they take passport photos).
Find a MAILBOXES ETC near you.
5.
STATEMENT OF MUTILATION
   

Fill in the Statement of Mutilation Form. This explains to the passport office how your passport became damaged.

MUTILATED PASSPORT

Include the damaged passport. Note that damaged passports are NOT normally returned to the applicant.

7.
PROOF OF TRAVEL
To expedite your passport you must provide one of the following to show your travel plans.
   
Copy of airline tickets, reservations, e-tickets, cruise tickets, or itinerary showing possible travel plans within the next 2 weeks to an international destination. The travel plans must have the applicants name printed on them.
A letter from the business or organization arranging the trip requesting an expedited passport from the passport agency. Here is an example of how the letter could be worded. PDF, WEB
8.
AUTHORIZATION LETTERS

 

Two copies of THE SAME authorization form filled in and signed. These letters show your acceptance facility what service you are using to get your passport expedited and allows them to release the "AA Sealed Envelope" (Details about the Acceptance Agent will be explained in another step). Please CALL OUR OFFICE at 1-866-ITS-EASY (1-866-487-3279) and one of our agents will instruct you to print out the correct authorization letter.

GOVERNMENT FEES
Prepare two checks or money orders for the Federal Government.
The fee that the U.S. Passport agency charges can be paid by check or money order ONLY. Personal and company checks are both fine. The check cannot be a starter check, that is your check must have a name preprinted on it.
One check / money order for $135.00 payable to "U.S. Department of State"

One check / money order for $25.00, your local acceptance agent will tell you how they would like this check made out.

   
INSTRUCTIONS
   
1.
SET ASIDE TRAVEL PLANS AND AUTHORIZATION LETTER

Set aside 1 copy of your travel plans and 1 of the authorization letters. You MUST send these to us when you ship your documents, which will be explained in a future step.

2.
GO TO YOUR LOCAL ACCEPTANCE AGENT
   

The applicant (and witness if you were instructed to take one) must appear in person at a local Acceptance Agent (AA) with all the requirements listed above . An acceptance agent is a government official who has been authorized to verify signatures for passport applications. AAs are usually a local post office or county clerk in your neighborhood. You can locate your nearest AA by going to the following link: FIND YOUR LOCAL AA

Inform the AA that you are using an expediting service to "HAND CARRY" your passport application to the U.S. Passport Agency for processing the following morning.

The envelope must be delivered to the Passport agency within 5 business days of the date the application was sealed.

After signing the DS11 application and witnessing you sign the application, the AA should enclose all the required documents (each requirement listed in the requirement section above) into a postal sealed envelope, which we call an "AA Sealed Envelope" and give the envelope back to you.

If an agent is unfamiliar with expediting services he/she may not want to release the AA Sealed Envelope to you, if you encounter this problem suggest that he/she refer to their Passport Agent's Manual on the topic "Hand-carrying of Executed Applications" it should be around page 31 of their manual.

   
NOTES

The acceptance agent is not the agent who approves passport applications, they are simply authorized to witness your signature and examine your documents. So if the acceptance agent tells you that certain documentation is not necessary and does not seal it up into the envelope, then you should still send us that documentation along with the sealed envelope. If the passport agency states the documentation is needed we will have it, if you don't include it you will be losing time and money getting us the required documents.

3.
PREPARE ENVELOPE

On the outside of the "AA Sealed Envelope" write down the applicant's:

Last Name, First Name
Date of Birth
Departure Date
Best Phone Number to Contact Someone at

     
If there is more than one application in the envelope write out each applicants name and date of birth. It does not matter where on the envelope this information is written.
Attached to the outside of the envlope you must include 1 copy of the letter of authorization that the It's Easy staff member directed you to print and sign, and a copy of your travel plans. (The set that you put iside in instruction 1).
4.
IT'S EASY ORDER FORM
Please fill in and include an It's Easy Order Form along with your order.

This form gives us at It's Easy the information about how to contact you, what service you are choosing, how you are paying our service fee and how we are shipping your application back to you.

It is IMPORTANT that the It's Easy order form is NOT sealed up inside the AA sealed envelope. If it is, fill out another one and send it along with the sealed envelope so we know how to process your application. (The It's Easy fees should not be sealed inside the envelope either).

5.
IT'S EASY SERVICE FEE

Include the It's Easy Service Fee, payable by Credit Card, Money Order or Check (no starter checks).

If you choose to pay the It's Easy Service fee by credit card you will fill in the credit card information at the bottom right of the It's Easy Order form.IMPORTANT: If your instructions required you to go to an acceptance agent make sure the It's Easy fees are NOT sealed inside the postal sealed envelope.
6.
RETURN SHIPPING

Include return Shipping. Choose one of the following:

N/C A Prepaid, filled in airbill from either FedEx, UPS, Airborn or DHL OR

7.
SHIP YOUR PACKAGE

Send all the previous requirements, the It's Easy order form, and if not included on the It's Easy Order Form, the It's Easy service fee and return shipping to the following address:

We STRONGLY recommend you send your documents by FedEx Priority Overnight. FedEx has the most reliable drop off times for courier services in the New York area. (They consistently deliver before 10:30am). We also STRONGLY recommend that you DO NOT ship your packages using Express Mail especially for next day / same day services. Express Mail consistently delivers to our office after 12, at which point it may not be possible to complete your order in a day.
8.
