	Your current passport has not been damaged in any way AND
	Your passport was issued for a term of less than 5 years (usualy a one year passport) AND
	Your passport was limited in validity because you lost your naturalization certificate.

REQUIREMENTS

1.	ONE APPLICATION FORM     If your passport was issued LESS THAN 1 YEAR AGO use the DS5504 Click HERE to download a DS5504 If your passport was issued OVER 1 YEAR AGO use the DS82 Click HERE to fill in the DS82 Online (Perferred) OR Click HERE to download a blank DS82.
2.	MOST RECENT PASSPORT     Include your previous U.S. Passport issued for passport that has not been damaged in any way. This must be the original.     If you are CHANGING YOUR NAME click HERE to find out what additional documents are needed to update your passport.
3.	PROOF OF CITIZENSHIP     Proof of U.S. Citizenship. You will need to provide one of the following. Previous U.S. Passport issued for full term (5 to 10 years) that has not been damaged in any way. OR Original Naturalization Certificate OR If you still have not received your new Naturalization Certificate then you will need to submit   A Receipt from INS (Immigration and Naturalization Services) showing that you have re-applied for your citizenship papers AND   A Letter from INS stating that you are still a U.S. Citizen. Note that they do not hand this letter our automaticaly, you must ask for it. Sometimes you may even have to argue for it. Explaine to them that you are applying for your U.S. Passport and the Department of State requires a letter from INS confirming your citizenship. Unfortunatly there is no specific name for this letter. NOTES Photocopies and notarized photocopies are NOT accepted, all certificates must be originals
4.	PASSPORT PHOTOS Submit two identical passport photos taken within the past few months.     These should be 2x2, white background, unaltered pictures that meet the U.S. Governments PASSPORT PHOTO REGULATIONS.     Kinkos, AAA, and Mailboxes Etc are examples of stores that take passport photos. If you don't have one of these stores located near you, try looking in your local Yellow Pages under "Photographers - Passport". Find a KINKOS near you. (Be sure to ask if they take passport photos - now that Kinkos has merged with FedEx not all their locations take passport photos). Find a AAA near you (be sure to ask that they take passport photos). Find a MAILBOXES ETC near you.
5.	PROOF OF TRAVEL To expedite your passport you must provide one of the following to show your travel plans.     Copy of airline tickets, reservations, e-tickets, cruise tickets, or itinerary showing possible travel plans within the next 2 weeks to an international destination. The travel plans must have the applicants name printed on them. A letter from the business or organization arranging the trip requesting an expedited passport from the passport agency. 6.	AUTHORIZATION LETTER   One letter of Authorization form filled in and signed. This letter shows the U.S. Passport Office that we have permission to process your passport. Please CALL OUR OFFICE at 1-866-ITS-EASY (1-866-487-3279) and one of our agents will instruct you to print out the correct authorization letter.
7.	GOVERNMENT FEES Prepare a check or money order for the Federal Government. The fee that the U.S. Passport agency charges can be paid by check or money order ONLY. Personal and company checks are both fine. The check cannot be a starter check, that is your check must have a name preprinted on it. If your passport was issued LESS THAN 1 YEAR ago - the fee is $60:   One check / money order for $60 payable to "U.S. Department of State" If your passport was issued MORE THAN 1 YEAR ago - the fee is $135.00:   One check / money order for $135.00 payable to "U.S. Department of State"

3.	RETURN SHIPPING Include return Shipping. Choose one of the following: N/A A Prepaid, filled in Airbill from either FedEx, UPS, Airborn or DHL OR $22.00 For FedEx Returns & Handling OR $35.00 For FedEx Staturday Delivery. (Check with FedEx in your area for availibility and delivery times) OR $55.00 For FedEx First Overnight Delivery. (Check with FedEx in your area for availibility and delivery times). $35.00 Hold @ JFK Airport. N/A Hold @ Great Neck Office. N/A Hold @ 360 Lexington Ave. N/A Messenger will Pick Up. $195.00 For Airline Delivery.         Shipping fees can be paid along with the It's Easy Service fee by either Check, Money Order, or Credit Card. 4.	SHIP YOUR PACKAGE Send all the previous requirements, the It's Easy order form, and if not included on the It's Easy Order Form, the It's Easy service fee and return shipping to the following address: IT'S EASY.COM 360 LEXINGTON AVE AIRLINE TRAVEL CENTER NEW YORK, NY, 10017         We STRONGLY recommend you send your documents by FedEx Priority Overnight. FedEx has the most reliable drop off times for courier services in the New York area. (They consistently deliver before 10:30am). We also STRONGLY recommend that you DO NOT ship your packages using Express Mail especially for next day / same day services. Express Mail consistently delivers to our office after 12, at which point it may not be possible to complete your order in a day.
