ADULT PASSPORT RENEWAL - NAME CHANGE

This only applies if you are renewing your passport (adult applicants only).
You can only do this if you changed your name AFTER your last passport was issued. If your name was changed BEFORE your most recent passport was issued you must either keep using the name that is in that passport, or start fresh as an ADULT NEW PASSPORT.
   
EXAMPLES
1. I Got Married January 1996
  I Renewed My Passport in my Maiden Name February 1996
     
  You CANNOT renew your passport in the married name. If you are using your married name you should follow the steps for an ADULT NEW PASSPORT. If you are still using your maiden name then you can simply renew your passport again in the maiden name.
   
2. I Got Married January 1996
  I Renewed My Passport in my Maiden Name December 1996
     
  You CAN renew your passport - just include the marriage certificate as indicated below.
   
NEEDED DOCUMENTATION
If you had your name change through:
  MARRIAGE Include your original court / government issued marriage certificate. Religious certificates are NOT accepted.
  DIVORCE Provide the original divorce decree (You will get it back along with your passport)
  COURT Provide the original government issued name change documentation.
  OTHER You will need to follow the steps for an ADULT NEW PASSPORT instead of a passport renewal.
