Adding Visa Pages Requirements

Current Passport

Applicant must submit their current passport in order to have visa pages added. Passport will be returned with an additional 24 visa pages.

1.2

Proof of Travel

Applicant must submit a computer-generated itinerary from a travel agency or online booking agency showing departure from the U.S. in order to get new passport quickly.

If traveling on business and no itinerary is available, a letter on company letterhead indicating dates and purpose of travel MAY suffice. If applicant does not possess proper itinerary, please contact us for options.

Authorization Letter

Applicant must submit "Letters of Authorization" which allow ItsEasy Passport and its sister companies to submit the application to the US State Dept.

All four letters of authorization must be completed and included.

Government Fees

The US State Dept charges $142 for expediting additional visa pages.

This adding visa pages fee may be made by personal check (no starter checks), business check or money order made payable to the US State Dept. or may be combined with your service fee credit card payment to ItsEasy when filling out the order form below. If paying by check or money order, include applicants date of birth on memo line.

Adding Visa Pages Application (DS-4085)

Applicant must submit one completed adding visa pages application form (DS-4085). Adding pages application must be filled out via US State Dept web site link below. This information is not submitted to the government, it is used to create a bar coded adding visa pages application required to expedite the adding pages request; hand filled adding pages applications are not acceptable for expedited services. After filling out adding visa pages application online, print completed adding visa pages application as instructed.

DO NOT forget to sign and date the completed adding pages application once printed.

Note: Having trouble with the US State Department online passport application site? The State Dept site has been overloaded recently causing it not to function consistently. A paper version of the application is available here, however, applications completed using the paper form may experience slight processing delays.

Ship Documents to ItsEasy

Ship all adding pages passport documents to ItsEasy Passport for processing.

ItsEasy recommends our preferred shipping partner Federal Express (FedEx) for document shipping. When completing the online order form above, you will be offered the use of the ItsEasy FedEx account to send document to us. By selecting this service, you will be provided a pre-addressed, pre-paid FedEx air bill upon completion. For urgent processing, it is recommended you select the "First AM" option for sending documents to ItsEasy.
