Instructions & Application
To Add Visa Pages, Applicant Must Meet the Add Visa
Pages Qualifications:
Applicant has US passport is in his or her possession, AND;
Applicant's passport has one or more years validity remaining
AND;
Applicant's passport has few or no remaining blank visa pages.

1.1
Current
Passport
Applicant must submit their current
passport in order to have visa pages added. Passport will
be returned with an additional 24 visa pages.
1.2
Proof
of Travel
Applicant must submit a computer-generated
itinerary from a travel agency or online booking agency showing
departure from the U.S. in order to add pages quickly.
If traveling on business and no itinerary is available, a
letter on company letterhead indicating dates and purpose
of travel will suffice for adding pages. If applicant does
not possess proper itinerary, please contact us for options.

Authorization
Letter
Applicant must submit "Letters
of Authorization" which allow ItsEasy Passport and
it's sister company to submit the adding visa pages application
to the US State Dept. All letters of authorization must be
completed to ensure fastest processing.
The US State Dept charges $60 for
expediting additional visa pages.
This adding visa pages fee may be made by personal check
(no starter checks), business check or money order made payable
to the US State Dept. or may be combined with your service
fee credit card payment to ItsEasy when filling out the
order form below. If paying by check or money order, include
applicants date of birth on memo line.

Adding Visa Pages Application (DS-4085)
Applicant must submit one completed
adding visa pages application form (DS-4085). Adding pages
application must be filled out via US State Dept web site
link below. This information is not submitted to the government,
it is used to create a bar coded adding visa pages application
required to expedite the adding pages request; hand filled
adding pages applications are not acceptable for expedited
services. After filling out adding visa pages application
online, print completed adding visa pages application as instructed.
DO NOT forget to sign and date the completed
adding pages application once printed.
Note: Having trouble with the US
State Department online passport application site? The State
Dept site has been overloaded recently causing it not to function
consistently. A paper version of the application is available
here, however,
applications completed using the paper form may experience
slight processing delays.

ItsEasy Online Order Form
Applicant must submit an ItsEasy
order form which provides contact information, service request
and payment information.
Ship all adding pages passport
documents to ItsEasy Passport for processing.

document shipping. When completing the online order form above, you will be offered
the use of the ItsEasy FedEx account to send document to us. By selecting this
service, you will be provided a pre-addressed, pre-paid FedEx air bill upon completion.
