Lost US Passport Application
Process Overview
Applicant must gather new passport documents listed below to take
to an Acceptance Agent.
An Acceptance Agent is a government employee who will verify
the identity of the applicant.
Acceptance agents are at designated Post Offices and Court
Houses.
Applicant must indicate that he/she is using a hand carry service
to process their lost or stolen passport application. Acceptance
Agent will have the applicant sign the lost or stolen passport
application, seal documents in an envelope and return the sealed
envelope.
Sealed envelope with missing passport application is forwarded
to ItsEasy Passport for processing.
Take the Following Lost or Stolen Passport Items to the Acceptance
Agent
1.1
Proof
of Citizenship
To replace a lost or stolen passport,
applicant must bring an original or certified copy of a birth
certificate, naturalization papers or old passport.

1.2
Proof
of Identification
Applicant must bring a driver's
license or state-issued identification issued more than six
months ago to apply for a lost or stolen passport replacement.
Official government or military picture identification is
acceptable. If this documentation is not available, click
here for more options.
1.3
Passport Photos
Applicant must submit two (2) identical
passport photographs for lost or stolen passport replacement.
These may be obtained at many Post Offices, Kinkos, Mailboxes
Etc or similar locations. Poor photos are the most common
reason for delay in lost or stolen passport replacement issuance.
Note: If applicant is delivering
documents to our offices personally, passport photos can be
taken on-site.
1.4
Proof
of Travel
Applicant must submit a computer-generated
itinerary from a travel agency or online booking agency showing
departure from the U.S. in order to replace passport quickly.
If traveling on business and no itinerary is available, a
letter on company letterhead indicating dates and purpose
of travel will suffice for lost or stolen passport applications.
If applicant does not possess proper itinerary, please contact
us for options.

Authorization
Letter
Applicant must submit an original
"Letter of Authorization" which allows ItsEasy
Passport to submit the lost passport application to the US
State Dept.
The US State Dept charges $135
for lost or stolen passport expediting and $25 for verification
of lost passport applications by a lost passport Acceptance
Agent.
Both lost passport fees must be paid to Acceptance Agent
onsite. The lost passport fee of $135 may be paid by check,
cash, money order or debit card (credit cards are not accepted).
If paying by check, starter checks are not acceptable. Indicate
the applicants DOB in the memo line.For missing passport applications,
an Acceptance Agent fee of $25 will apply. This fee may be
paid via any payment method.

Missing
Passport Explanation Form (DS-64)
As part of the replacement process, applicant
must explain the circumstances regarding the recent loss or
theft using government form DS-0064.
On the application, applicant will be asked questions regarding
the loss of previous passport. Answer the questions clearly
and concisely as if speaking directly to a US State Dept.
official. Questions regarding your previous passport number,
expiration date, etc may be left blank if information is not
available
Lost
or Stolen US Passport Government Application (DS-11)
Applicant must submit one completed
lost or stolen passport application form (DS-11). Lost passport
application must be filled out via US State Dept web site
link below. This information is not submitted to the government,
it is used to create a bar coded lost passport application
required to expedite the replacement passport request; hand
filled missing passport applications are not acceptable for
expedited services. After filling out a lost or stolen passport
application online, print two (2) completed passport applications
- one copy will be included in the sealed envelope, the second
will be provided to ItsEasy.
DO NOT SIGN the completed application until
instructed to do so by Acceptance Agent.
Note: Having trouble with the US
State Department online passport application site? The State
Dept site has been overloaded recently causing it not to function
consistently. A paper version of the application is available
here, however,
applications completed using the paper form may experience
slight processing delays beyond our control.
Note:
It is imperative that you include a copy of your completed
passport application outside of the sealed acceptance
agent envelope. Applications received without copies
may experience delays beyond our control.
Complete Your Package to ItsEasy for

2.1
Lost
Passport Package Contents
Applicant must provide ItsEasy
with the sealed envelope from the Acceptance Agent containing
the lost or stolen passport replacement documents above.
On the outside of the sealed envelope, applicant must write
the following:
Last Name, First Name
Date of Birth
Departure Date
Contact Phone Number
* If there is more than one application in the envelope
(for multiple applicants), write out each applicants name
and date of birth.
Important:
ItsEasy cannot open the sealed envelope, therefore applicant
must submit copies of the following documents in their
package outside of the sealed envelope:
Completed (unsigned) New Passport Application
Flight Itinerary
Authorization Letter
Applications received without copies may experience
delays beyond our control.
2.2
ItsEasy Online
Order Form
Applicant must submit an ItsEasy order form
which provides contact information, service request and payment
information.
Ship all Lost or Stolen passport
documents to ItsEasy Passport for processing.

document shipping. When completing the online order form above, you will be offered
the use of the ItsEasy FedEx account to send document to us. By selecting this
service, you will be provided a pre-addressed, pre-paid FedEx air bill upon completion.
