Lost Child US Passport Application
Process Overview (16 & 17)
Applicant must gather new passport documents listed below to take
to an Acceptance Agent.
An Acceptance Agent is a government employee who will verify
the identity of the applicant.
Acceptance agents are at designated Post Offices and Court
Houses.
Applicant must indicate that he/she is using a hand carry service
to process their new passport application. Acceptance Agent will
have the applicant sign the new passport application, seal documents
in an envelope and return the sealed envelope.
Applicant may be required to show consent of one parent or legal
guardian in order to process the application - this is required
if applicant does not have valid government issued photo identification
issued more than six months ago. It is recommended that applicants
16 and 17 years old appear with one parent or legal guardian who
is capable of proving relation to the child by way of government
issued photo identification.
Sealed envelope with new passport application is forwarded
to ItsEasy Passport for processing.
Take The Following Lost Passport Items to the Acceptance Agent
1.1
Proof
of Citizenship
To obtain a new passport, applicant
must bring an original or certified copy of a birth certificate,
naturalization papers or old passport.
Birth certificates must show applicant and parent(s) full
names - "short form" birth certificates may not
be acceptable.

1.2
Proof
of Identification
Applicant must bring a driver's
license or state-issued identification issued more than six
months ago to apply for a new passport. Official government
or military picture identification is acceptable. If this

1.3
Passport Photos
Applicant must submit two (2) identical
passport photographs for new passports. These may be obtained
at many Post Offices, Kinkos, Mailboxes Etc or similar locations.
Poor photos are the most common reason for delay in new passport
issuance.
Note: If applicant is delivering
documents to our offices personally, passport photos can be
taken on-site.
1.4
Proof
of Travel
Applicant must submit two (2) copies
of a computer-generated itinerary from a travel agency or
online booking agency showing departure from the U.S.
One copy must be included in the sealed envelope and a second
copy included in the package to ItsEasy.
If traveling on business and no itinerary is available, a
letter on company letterhead indicating dates and purpose
of travel will suffice for new US passport applications. If
applicant does not possess proper itinerary, please contact
us for options.

Authorization
Letter
One of applicants parents or legal
guardians must submit an original "Letter of Authorization"
which allow ItsEasy Passport to submit the passport application
to the US State Dept. The link below will provide you with
two copies.
One will go in the sealed envelope, the second (watermarked
"EXTRA COPY") will be included in the package to
ItsEasy.
The US State Dept charges $170
for new passport expediting for children 16 and 17 years old
and $25 for verification of application by a passport Acceptance
Agent.
Both new passport fees must be paid to Acceptance Agent onsite.
The new passport fee of $170 may be paid by check, cash, money
order or debit card (credit cards are not accepted). If paying
by check, starter checks are not acceptable. Indicate the
applicants name and DOB in the memo line. For new passport
applications, an Acceptance Agent fee of $25 will apply. This
fee may be paid via any payment method.
1.7
Missing Passport
Explanation Form
As part of the replacement process, applicant
must explain the circumstances regarding the recent loss or
theft using government form DS-0064.
On the application, applicant will be asked questions regarding
the loss of previous passport. Answer the questions clearly
and concisely as if speaking directly to a US State Dept.
official. Questions regarding your previous passport number,
expiration date, etc may be left blank if information is not
available
Applicant must submit one completed
new passport application form (DS-11). Application must be
filled out via US State Dept web site link below. This information
is not submitted to the government, it is used to create a
bar coded application required to expedite the new passport
request; hand filled passport applications are not acceptable
for expedited services. After filling out a new child passport
application online, print two (2) completed passport applications.
One copy must be included in the sealed envelope and a second
copy (which does not need to be signed) included in the package
to ItsEasy.
Note: When completing the application,
you may be asked to select services including additional shipping.
It does not matter what you select as these fees will be paid
as part of the service you are selecting from ItsEasy.
DO NOT SIGN the completed application until
instructed to do so by Acceptance Agent.

Complete Your Package to ItsEasy for New Passport Processing
2.1
New
Passport Package Contents
Applicant must provide ItsEasy
with the sealed envelope from the Acceptance Agent containing
the new passport documents above.
Important: Most standard
processing passport applications are submitted through the
post office. The post office encourages applicants to pay
for fast shipping which adds between $15 and $25 to the cost
of the passport. Since you are using an expediting service,
DO NOT PAY THE ADDITIONAL POSTAGE FEES. This will cause delay
and possible rejection of your expedited passport application.
You will pay two fees at the post office, $25 for the Acceptance
Agent and $170 for the passport. DO NOT PAY FOR ADDITIONAL
POSTAGE.
On the outside of the sealed envelope, applicant must write
the following:
Last Name, First Name
Date of Birth
Departure Date
Contact Phone Number
* If there is more than one application in the envelope
(for multiple applicants), write out each applicants name
and date of birth.
Important:
ItsEasy cannot open the sealed envelope, therefore applicant
must submit copies of the following documents in their package
outside of the sealed envelope:
Completed (unsigned)
New Passport Application
Flight Itinerary
Authorization Letter

Applicant must submit an ItsEasy order form
which provides contact information, service request and payment
information.
NOTE:
"Processing" refers to business days and does not

2.3
Ship
Documents to ItsEasy
Ship all child passport documents
to ItsEasy Passport for processing.

ItsEasy recommends our preferred shipping partner Federal Express (FedEx) for
document shipping. When completing the online order form above, you will be
offered the use of the ItsEasy FedEx account to send document to us. By selecting
this service, you will be provided a pre-addressed, pre-paid FedEx air bill
upon completion. For urgent processing, it is recommended you select the "First
AM" option for sending documents to ItsEasy.
