Passport Name Change Requirements

Current Passport

Applicant must submit their current passport. Passport will be returned marked "Cancelled" by the US State Department along with new passport reflecting name change.

Passport Photos

Applicant must submit two (2) identical passport photographs for passport renewals. These may be obtained at many Post Offices, Kinkos, Mailboxes Etc or similar locations. Poor photos are the most common reason for delay in passport issuance.

Note: If applicant is delivering documents to our offices personally, passport photos can be taken on-site.

Name Change Documentation

Applicant must submit original or certified copy of the marriage certificate (not a license) or a court decree ordering a name change or an adoption. The document must show the official seal of the issuing authority. All official documents will be returned.

Proof of Travel

Applicant must submit a computer-generated itinerary from a travel agency or online booking agency showing departure from the U.S. in order to update name quickly.

If traveling on business and no itinerary is available, a letter on company letterhead indicating dates and purpose of travel will suffice for name changes. If applicant does not possess proper itinerary, please contact us for options.

Authorization Letter

Applicant must submit "Letters of Authorization" which allow ItsEasy Passport and it's sister company to submit the name change application to the US State Dept. All letters of authorization must be completed to ensure fastest processing.

Government Fees

The US State Dept charges $135 for expediting name change applications.

This name change fee may be made by personal check (no starter checks), business check or money order made payable to the US State Dept., or may be combined with your service fee credit card payment to ItsEasy when filling out the order form below. If paying by check or money order, include applicants date of birth on memo line.

Name Change Application (DS-82)

Applicant must submit one completed name change application form (DS-82). Name change application must be filled out via US State Dept web site link below. This information is not submitted to the government, it is used to create a bar coded name change application required to expedite the name change request; hand filled name change applications are not acceptable for expedited services. After filling out name change application online, print completed name change application as instructed.

DO NOT forget to sign and date the completed name change application once printed.

Note: Having trouble with the US State Department online passport application site? The State Dept site has been overloaded recently causing it not to function consistently. A paper version of the application is available here, however, applications completed using the paper form may experience slight processing delays.

Ship Documents to ItsEasy

Ship all passport name change documents to ItsEasy Passport for processing.

ItsEasy recommends our preferred shipping partner Federal Express (FedEx) for document shipping. When completing the online order form above, you will be offered the use of the ItsEasy FedEx account to send document to us. By selecting this service, you will be provided a pre-addressed, pre-paid FedEx air bill upon completion.
