Second Passport Requirements,
Instructions & Application
To Get a 2nd US Passport, Applicant Must Meet Duplicate
Passport Qualifications:
Applicant has a valid US passport is in his or her possession,
AND;
Applicant was 16 years or older when passport was issued AND;
Applicant wishes to secure a duplicate, limited validity passport.
1.1
Current
Passport(s)
If applicant is applying
for a duplicate passport for the first time; applicant
must submit valid 10 year passport in order to apply for a
second passport.
If applicant is applying to extend or renew their
current duplicate passport; applicant must submit
both their current 10 year passport AND their current duplicate
passport.
All passports submitted for duplicate passport application
will be returned.
1.2
Drivers License
Photocopy
Applicant must submit a 200% photocopy of their
current drivers license. Both the photo and text must be clear
and legible.
1.3
Second
Passport Photos
Applicant must submit two (2) identical
passport photographs for second passport. These may be obtained
at many Post Offices, Kinkos, Mailboxes Etc or similar locations.
Poor photos are the most common reason for delay in passport
issuance.
Note: If applicant is delivering
documents to our offices personally, passport photos can be
taken on-site.

Proof
of Travel
Applicant must submit a computer-generated
itinerary from a travel agency or online booking agency showing
departure from the U.S. in order to get second passport quickly.
If traveling on business and no itinerary is available, a
letter on company letterhead indicating dates and purpose
of travel will suffice for duplicate passports. If applicant
does not possess proper itinerary, please contact us for options.

Authorization
Letter
Applicant must submit "Letters
of Authorization" which allow ItsEasy Passport and
it's sister company to submit the second passport application
to the US State Dept. All letters of authorization must be
completed to ensure fastest duplicate passport processing.
Applicant must submit a letter addressed to
the U.S. State Department explaining why he/she is requesting
a second passport. The most common requests for duplicates
are due to frequent international travel and the need to obtain
visas while out of the US or traveling to countries which
do not approve of another countries visa or entry stamp.
Second passport request must be written by applicant, third
party letters do not suffice.

Government
Fees
The US State Dept charges $135
for expedited duplicate passports.
This second passport fee may be made by personal check (no
starter checks), business check or money order made payable
to the US State Dept., or may be combined with your service
fee credit card payment to ItsEasy when filling out the order
form below. If paying by check or money order, include applicants
date of birth on memo line.

Duplicate
US Passport Government Application (DS-82)
Applicant must submit one completed
duplicate passport application form (DS-82). Second passport
application must be filled out via US State Dept web site
link below. This information is not submitted to the government,
it is used to create a bar coded duplicate application required
to expedite the second passport request; hand filled duplicate
passport applications are not acceptable for expedited services.
After filling out second passport application online, print
completed second passport application as instructed.
DO NOT forget to sign and date the completed
duplicate application once it is printed.
Note: Having trouble with the US
State Department online passport application site? The State
Dept site has been overloaded recently causing it not to function
consistently. A paper version of the application is available
here, however,
applications completed using the paper form may experience
slight processing delays.

Applicant must submit an ItsEasy
order form which provides contact information, service request
and payment information.
Ship all duplicate passport documents
to ItsEasy Passport for processing.

ItsEasy recommends our preferred shipping partner Federal Express (FedEx) for
document shipping. When completing the online order form above, you will be offered
the use of the ItsEasy FedEx account to send document to us. By selecting this
service, you will be provided a pre-addressed, pre-paid FedEx air bill upon completion.
